By signing up for coaching services, purchasing a program, or submitting your information, you acknowledge and agree to the following terms:
Charlie Huff Health Coaching LLC provides wellness and health coaching services only and does not provide medical diagnosis, treatment, psychotherapy, or emergency services.
Information you provide may include personal wellness goals, habits, lifestyle history, nutrition patterns, and related information used to support coaching services.
Charlie Huff Health Coaching takes reasonable steps to protect your personal information; however, coaching businesses may not be a “covered entity” under the federal HIPAA law unless operating in connection with a healthcare provider, insurer, or covered entity.
Because standard email, text messaging, and certain scheduling systems may not be fully encrypted, you acknowledge there are inherent privacy risks in electronic communication.
Please do not submit highly sensitive medical records, diagnostic reports, insurance information, or urgent medical concerns through this form unless specifically requested.
If you are experiencing a medical or mental health emergency, call 911 or contact an appropriate licensed healthcare provider immediately.
You consent to be contacted regarding scheduling, coaching services, follow-up communication, and administrative matters.
You may request updates or deletion of your information subject to legal and business recordkeeping obligations.
